I think there was a helicopter pilot killed filming a Magnum PI episode as well.
The first one comes to mind is the Twilight Zone tragedy. Not only a good actor was killed, but two very young and adorable children were killed (see Scott's write up on Vic Morrow). However, the girl has something on her face that looks a little creepy. It probably isn't but just suggestion after one knows what happens to these two angels.
I guess the Twilight Zone tragedy comes to mind because a helicopter is involved. The helicopter is not my favorite type of aircraft. My husband is a software engineer working on flight simulators. I hate when he has to go up in a helicopter. Once we were passing through a town north of here and he said, "Hey, I flew up here this week on the EC-135." I'm here, "YOU DIDN'T me that!!!" Probably a good thing because I'd be worried spitless.

I watched a video on set disasters back in 2014 they were filming a Greg Allman biopic movie and the scene called for a hospital bed on a train track, they didn't really have a warning sign for the train coming down on them and a production assistant was struck by the hospital bed which knocked her in front of the train.
